How to register
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by completing a form available from reception.
We recommend that new patients undertake a health check with a Healthcare Assistant.
Appointments
We run all our surgeries by an appointment only system. If you want to book an appointment at a particular time, or if you want a choice of doctor or nurse you can book upto 2 weeks ahead. You can either telephone, book in person or book online. If you are too ill to come to the surgery and you require a visit at home, please call the surgery before 10am. The Receptionist will take some details and then arrange for the Triage Nurse to call you back. Home visits take up much more of a doctor’s time than a consultation in the surgery, so if you are mobile please come into the surgery to be seen.

Hypertension Clinic
|
Patients who are found to be hypertensive on three occassions are invited annually to review their blood pressure and medication. | |
| Diabetes Clinic | All patients with diabetes are automatically invited to the clinic periodically. Patients who are experiencing difficulties with their diabetic control are seen more frequently | |
| Vaccinations |
We run a flu jab clinic in late October. |

| Asthma | Regular check-ups and advice from practice nurses on lifestyle, inhaler techniques and medication. | |
| Phlebotomy Clinic | Blood tests are taken by our Phlebotomists (or by the district nurses for strictly housebound patients only). Blood tests can only be taken on weekday mornings in order that the sample can be transported to the laboratory by 10.45am. | |
| Contraceptive Services | We offer a range of contraceptive services including the pill and the injection. Please arrange an appointment with the Practice Nurse. | |
| Cervical Cytology Clinic | All women between the ages of 20 and 64 are encouraged to have a cervical smear. These are usually carried out every 3 years or more frequently if previous smears suggested this would be more appropriate. | |
| INR Clinic | INR Clinics are run by out Health Care Assistants under the guidelines issued by NICE. With the latest near patient testing equipment we are able to provide instant results with follow up appointments for the next test. | |
| Child Health Surveillance | The Health Visitors are based in Ala Road Health Unit and visit the surgery regularly. They are responsible for administering immunisations from 0-5 years. | |
| Travel Vaccinations Clinic | Patients travelling abroad should obtain a form from Reception prior to making an appointment with the Practice Nurse. Patients are advised to attend at least four weeks before date of departure. |
Repeat prescriptions
Please leave your request for a repeat prescription at reception or post it to the surgery. Tick the items you need on the right-hand section of your previous prescription form. You can also order your prescription online. Your prescription will be ready for collection 48 hours later.
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
Sick certificates
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
Test results
We have a system to give out test results between 9.00am and 11.30am. To obtain any results please ring the surgery and press option 2 for test results - you will be directed to a mailbox where you should leave your name, date of birth and telephone number and the nurse will call you back before 11.30am.
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
Non-NHS services
The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the surgery.
